How Social Security evaluates fibromyalgia

Fibromyalgia has no Blue Book listing, which is why so many claims fail. SSA evaluates it under a dedicated ruling, SSR 12-2p, which requires a licensed physician's diagnosis plus either widespread pain with tender points, or widespread pain with repeated manifestations of six or more fibromyalgia symptoms — and, critically, evidence that other disorders which could cause the symptoms have been excluded. The exclusion work is the part claimants most often have not had done.

The evidence that actually decides it

Examiners work from documents, not from how you feel on the day. These are the records that carry the most weight in a fibromyalgia claim:

• A physician's diagnosis meeting the ruling's criteria, not a self-report
• Documentation that other causes were investigated and ruled out
• A longitudinal record — fibromyalgia claims are won on consistency over time
• Notes on fatigue, cognitive difficulty, and non-restorative sleep

Why fibromyalgia claims get denied

Most denials are not a judgement that you are able to work. They are gaps in the file. The recurring ones here:

• No documented workup excluding other conditions, as the ruling requires
• Normal objective testing treated as evidence of no impairment
• A short treatment history, where the record cannot show persistence

If a claim was denied for one of these, appealing is almost always better than starting over — you have 60 days from the date on the letter, and appealing preserves the filing date your back pay is calculated from.

What you can still do, and why it matters most

Sustainability rather than a single-moment capability — whether you could maintain attendance and pace five days a week, not whether you can do a task once.

This is what Social Security calls residual functional capacity, and it decides more claims than the listings do. Two people with the same diagnosis routinely get opposite decisions, because the question was never the diagnosis.

When a claim is approved, benefits are often owed back to an earlier date and paid as a lump sum on top of the monthly amount. How far back that reaches depends heavily on when you filed.

Every month you postpone starting a claim is potentially a month of back pay you don't get. If a condition has already kept you out of work, the filing date is the thing worth protecting first.

No recent work history doesn't automatically rule you out

There are two programs and people accidentally rule themselves out of both. SSDI is built on work credits. SSI is built on financial need and doesn't require recent work at all — and in most states it brings Medicaid with it.

A homemaker, someone unemployed for years, or a worker whose credits have lapsed may still have a path. “I haven't worked in a long time, so there's no point” is the single most common reason people never apply, and it's frequently wrong.
